PLEASE READ MMCS' Privacy Policy and Camp Rules

On behalf of my child(ren), I accept and assume any and all risks associated with his/her/their attendance and participation in the camp and its activities. I understand that my child(ren) should not attend the camp if he/she/they are not healthy. I understand that my child(ren) must abide by camp policies and the instructions of the camp staff. I agree that should my child be dismissed from camp no part of my tuition will be refunded. I understand that no reduction in the tuition will be made for late arrival, early departure, vacations, illness or injury. In the event that I cannot be contacted in an emergency, I hereby grant Mentor Me Community Support permission to give immediate treatment and/or take my child to a hospital emergency room. Permission is hereby granted for photographs and/or videos to be taken of my child at camp and Mentor Me Community Support has the right to utilize these in brochures, videos, slide shows, web site, and other camp materials. Permission is also granted for my child(ren) to attend all scheduled field trips. Knowing these facts, and in consideration of you accepting my child(ren)’s application, I, for myself, my child(ren) attending the camp, and anyone else who might claim on my or my child(ren)’s behalf (”I”), hereby agree that Mentor Me Community Support is not responsible for accidents, injuries, and/or medical or dental expenses arising from my child(ren)’s participation in the camp and, accordingly, I covenant not to sue, and waive, release, and discharge Mentor Me Community Support, and anyone working on their behalf from any and all claims of liability or expenses of any kind or nature whatsoever arising out of, or relating to my child(ren)’s participation in the camp. I have carefully read all of the information in this application form and agree to all conditions.

Privacy on the Internet is important to you and all of us at MMCS. As part of the operations of MMCS, we gather certain types of information about MMCS users.  This Privacy Policy ("Privacy Policy") explains the types of information we gather, what we do with it and how we correct or change information that MMCS may collect.

Refund Policy:

All cancellation must be received before June 1st and a non refundable $75.00 registration fee is due prior to registration. Cancellations must be received in writing by email at: info@mentormecs.com or by U.S. mail (P.O. Box 61422 King of Prussia, PA 19406) No refunds will be made for requests received after June 1st. Refunds will be issued in the same form payment was made. Please allow two weeks for processing. No Information will be sold to 3rd parties.

Information Collected by MMCS

MMCS collects two types of information about users: personal information (such as name and e-mail address) and aggregate information (such as information about how many users log on to our sites on a daily basis).

Personal Information

Personal information is collected when users voluntarily register for information, such as your comments on the content or design of the web site.

Aggregate Information

One method of collecting aggregate information about your activities online (such as the pages you access most frequently or the search terms you enter) and information volunteered by you (such as survey or registration information) is by tracking Internet Protocol (IP) addresses. An IP address is a number that is automatically assigned to your computer whenever you surf the Web. The computers that deliver the Web pages to you automatically identify your computer by its IP address. When you request a page from one of our sites, our servers log your IP address. Our sites record IP addresses for the purposes of system administration and to provide aggregate data to advertisers about the volume of use on our sites.

Use of Information Collected by MMCS

MMCS uses the personal information we collect to improve the content of our sites, to customize the content and/or layout of our sites for each individual user, and to notify users about updates to our sites or promotional offers or opportunities that we believe will be of interest to users.

Additional Information

No transmission of data over the Internet is guaranteed to be completely secure. It may be possible for third parties not under the control of MMCS to intercept or access transmissions or private communications unlawfully. While we strive to protect your personal information, MMCS cannot ensure or warrant the security of any information you transmit to us. Any such transmission is done at your own risk.

Under no circumstances, including but not limited to negligence, shall MMCS be liable for any special, incidental or consequential damages that result from the use of the information or materials on our sites, even if MMCS or an authorized representative of MMCS, has been advised of the possibility of such damages. This section also should be read in conjunction with the full Terms of Service.
